A possible explanation of the process: - The training takes place in VR. - During it, the user goes through a simulated patient visit in a hospital room. - During the visit, it is checked whether the rules of hand hygiene are applied correctly. - First, the user goes through a [Tutorial](https://wiki.vrhandhygiene.com/ai/en/app/tutorial.md). There the controls are explained and the most important interactions are practised. - One or more patient visits then follow. - After individual visits or at the end of the training, the mistakes made can be analysed and discussed together in the replay system. ### Introduction to the VR hardware Before starting in VR, the operator explains the most important controls to the user: The operator shows how to hold the controllers, where the relevant buttons are located, and which actions can be performed with them. They then help the user put on the VR headset and explain how to position it correctly and adjust it if needed. The goal is for the user to wear the VR headset comfortably, see the display clearly, and operate the controllers confidently. ### Accompanying the tutorial The operator starts the tutorial and accompanies the user during the introduction. There the user learns the basic controls as well as the most important work steps of the patient visit. If the user has difficulties, the operator can provide verbal support. If necessary, a tutorial step can be restarted or skipped. ### Supervising the visit After the tutorial, the operator starts the patient visit. During the visit, they observe the user and only step in when necessary, for example in case of technical problems or when the user does not understand an interaction. The operator makes sure that the user carries out the tasks independently. Depending on the training concept, professional guidance on hand hygiene can be given during the visit or only in the debriefing afterwards. ### Analysing and discussing mistakes After a visit, the operator can use the replay system to analyse the run together with the user. Critical situations such as indication errors, missing hand disinfection, or germ transmission can be examined and discussed in a targeted way. The replay system is used to make mistakes comprehensible and to clearly explain the connection between action, hygiene rule, and consequence. ### Concluding the training After the final run, the operator helps the user take off the VR headset and put down the controllers. A short debriefing can then take place outside of VR. During this, open questions can be clarified, observations summarised, and key learning points repeated. To close, the operator says goodbye to the user. ## User interface during the visit During the visit, the operator sees the user's view in VR on the screen. In addition, a user interface is overlaid that displays information about the ongoing visit and offers various control options. The user interface consists of two side panels. The left panel shows information about indications and indication errors. The right panel shows the progress of the visit and contains the most important controls. Indication errors are highlighted so that the operator can recognise them quickly. With the Filter indications switch, you can specify which entries are shown. If the switch is enabled, only indication errors are shown. If it is disabled, all indications are shown. After ending a visit, the operator can switch between Score and Replay in the Evaluation area. This allows the completed run to be analysed either based on the results or step by step in the replay system. As long as the visit has not yet ended, the buttons are disabled. #### Explanation of indications In the left panel, all relevant contacts are logged as indications during the visit. An **indication** occurs in ViRTUE when two objects, body parts, or hands touch. This can be, for example, contact between the user's hand and a patient, an object in the patient room, or a medical instrument. An **indication error** occurs when two different zones are involved in a contact without a prior hand disinfection. The assessment is based on the [zone model](https://wiki.vrhandhygiene.com/ai/en/visit/hand-hygiene.md#zonenmodell). An entry in the indication list is structured as follows: - The first line shows whether it is a normal indication or an indication error. It also shows at which point during the visit the contact occurred. - Below that, the two objects involved are listed. The first line names the object, for example hand right (user) or Duvet (Patient 1). - The label in parentheses serves only to distinguish objects that occur more than once. It must not be used to automatically infer the zone of the object. An object labelled Patient 1 therefore does not necessarily belong to the Patient 1 zone. In practice, however, this is almost always the case. - The second line indicates which zone the object was assigned to at the time of the contact. This assignment can be adjusted for each object in the [Settings](https://wiki.vrhandhygiene.com/ai/en/app/germs-indication-zones.md). For certain objects, the zone assignment can change during the visit. These include, for example, the user's hands or the thermometer filter caps. This allows the list to make clear which zones were actually involved in each contact. ### Right panel In the right panel, the operator sees the current progress of the visit. The **timer** shows the remaining time for the current run. It is only displayed if scoring is enabled for the visit. Under **Tasks**, the tasks to be completed for each patient are displayed. Tasks already completed are shown with blue icons. This allows the operator to see at any time which work steps the user has already finished. Using the **Germ and indication zone view**, the operator can specify which visualisation is shown in the scene. From left to right: - **None:** Neither germs nor indication zones are visualised. - **Germs:** Only germs are visualised. - **Indication zones:** Only indication zones are visualised. - **Germs and indication zones:** Both visualisations are shown at the same time. > **TIP**: The user interface can be shown and hidden with the key combination `CTRL + H`. - **End visit:** The operator can end the currently running visit early. This button is only shown while the visit is still active. - **Next visit:** Starts the next run. This button is only shown after the current visit has ended. - **Main menu:** Ends the current visit as well as all remaining visits of the training. The main menu then opens again. > **TIP**: Before starting the next visit, the operator should remind the user to go to the hospital corridor. ## User interface after the visit The visit is considered ended as soon as the user has completed all tasks for both patients. They can then remain in the patient room for a while. During this time, the operator can discuss the results with the user verbally and/or start a replay of the visit in parallel, where the moments at which mistakes happened can be reviewed. ## Score After a visit has been completed, the score appears in the centre of the screen, provided it was enabled for the visit. It is shown in the middle, between the left and the right panel. The buttons in the Evaluation area of the left panel are now active. The operator can switch between Score and Replay. The replay mode starts a playback of the visit that has just been completed. This allows the run to be analysed together with the user. The remaining functions of the left and right panels remain available. The score view shows the following information: - **Stars:** Shows how many stars the user achieved. From 0 to 3 stars are possible. - **Score:** The score overview shows how the score is composed: - **Maximum score:** the maximum possible score for the visit. - **Indication errors:** the number of indication errors made and the points deducted as a result. - **Sick patients:** the number of infected patients and the points deducted as a result. - **Timeouts:** the duration of the time exceeded and the points deducted as a result. - **Total:** the total score achieved after all deductions. Since several visits can be completed one after another during a training, the evaluation shows at the top centre which visit the currently displayed results refer to. Using the arrow buttons on the left and right, the operator can switch between the evaluations of the individual visits. These recordings can be viewed again directly after a visit has been completed or at a later point in time. This re-viewing is referred to as replay. The replay is used to analyse specific situations in a targeted way and to discuss them together with the user. These include, for example, indication errors caused by missing hand disinfection. In this way, the actions performed, any mistakes, and the professional background can be linked directly with one another. > [**Image**](https://wiki.vrhandhygiene.com/_astro/operator-replay.en.Cm-6gzf1_Z2sHMki.webp): Replay mode from the operator's perspective > > _Operator in replay mode_ To switch to replay mode, the operator clicks Replay under Evaluation in the left panel. The recording is then loaded. This can take a few seconds. The operator sees the replay from the user's perspective during the recorded visit. Clicking again pauses the replay. While the replay is paused, the operator can rotate their view freely. To do so, they click into the view with the mouse and drag in the desired direction. As soon as playback resumes, the perspective switches back to the user's original viewing direction during the recording. Using the timeline, the operator can jump to any point in the recording. To do so, they click on the desired position in the timeline or drag the playback marker to the desired position. In addition, the operator can navigate directly via the indication list. When an entry in the list is clicked, the replay automatically jumps to the moment just before the corresponding indication or indication error. This function is available both during ongoing playback and in the paused state. > **TIP — Tips for the replay**: > > - The Germ and indication zone view can also be changed during the replay. The operator can therefore decide at any time whether to show no visualisation, only germs, only indication zones, or both visualisations at the same time. > - At the top centre, the operator can switch between the evaluations of several visits in the same training. If three visits were carried out, for example, the operator can select which visit should be evaluated or played back as a replay. > - This also makes it possible to start the replay of the first visit again after the third visit has been completed. The operator is free to decide which visit they want to discuss together with the user.
